Setting the form destinations

Configure how and where the information is delivered at the end of a Smart Form session.

What happens when respondents click the Submit button? That depends on the destinations you’ve chosen for the form. Use destinations to decide (1) what respondents see when they click Submit, and (2) where the form results are sent.

Tip  Make sure you’ve defined the documents that will be created with the form results.

Open the Submission panel

1. Click the Submission panel, then click EDIT NODE.

What should FormHero do when your user submits the Smart Form?

Decide what respondents will see when they click Submit.

Just Thank the User

This option sends respondents directly to the Thank You panel.

Get eSignature then Thank the User

This option requires respondents to provide an electronic signature, then sends them to the Thank You panel. Select this option if you’re creating a PDF that requires a signature.

Tip  Make sure your PDF template includes a signature field.

Select eSignature Provider
Select the eSignature company you want to use.
Document/Package Description
Enter the email subject.
Recipient Name, Recipient Email
Enter the name and e-mail address for the recipient of the signed PDF.

Send an E-mail then Thank the User

This option lets you send an email with the form results, then send respondents to the Thank You panel. You could use this option to send the results for approval, kick off another process or simply email the person who completed the form.

Tip  Respondents are sent to the Thank You panel without any indication that an email has been sent. You can add a message to the Thank You panel to let respondents know if you’ve sent them an email.

Recipients, CC Recipients, BCC Recipients 
Click Add Recipient and enter the recipient’s Name and E-Mail Address.
From Name 
Name of the person sending the e-mail.
From E-mail Address 
E-mail address of the person sending the email.
E-mail Subject 
Subject line.
HTML Message Content 
Message. You can use HTML tags to format the message.
Text-Only Message Content (to support non-HTML e-mail clients) 
Enter a text version of the message. The text version will be used if the recipient’s email client doesn’t support HTML.
Select Outputs to Include as Attachments 
Include any of the form outputs as an attachment. For detailed instructions, see Select Outputs to Include as Attachments.

Allow user to View/Print/Download the Resource then say Thank You

Available for PDF documents only. This option shows respondents the form results on the Thank You panel. Respondents can print and download the results using their web browser.

Enter instructions for downloading the results, such as “Please download a copy of the form for your records.”

Select Outputs to Include as Attachments

You can add any of the form outputs (such as a PDF) as an attachment to the email.

  1. Click Select Outputs to Include as Attachments.
  2. Select the checkbox beside the outputs you want to include as attachments.

You can choose to only include an attachment under specific conditions. For example, you might want to include an attachment if the respondent answered yes to a question or if the respondent lives in a specific area. For example, the following condition states that the attachment will only be included if the respondent’s country is Canada.

Click ALWAYS, then select WHEN.

Choose whether ALL or ANY of the conditions must be true.

For each condition, select + add condition and enter the following information:

Select a data binding
This is the data binding you want to check. In our example, the data binding is client.country.
Select an operator
Decide how to evaluate the data binding value. In our example, the operator is is equal to.
Enter a value
Enter the value you’re checking for. In our example, the value is Canada.
Ignore Case
Select this option if it doesn’t matter if the value contains uppercase or lowercase letters. We’ve selected this option in our example, so the values Canada and canada would both meet the condition.

Forward the form results

Use Additional Actions to choose where the results are sent when respondents click Submit. You can add as many actions as you want.Call a Webhook

Use a webhook to send the results to another application.

  1. Click EDIT and enter the webhook information.
  2. To test the settings, click MAKE A TEST CALL.

Send an E-Mail

Send an e-mail and attach the form results. Click EDIT and enter the following information.

SMTP Configured
Enter the name of the SMTP server in the testing and production environments.
Recipients, CC Recipients, BCC Recipients
For each recipient, enter their name and e-mail address, and select the environment that will be used to send the email.
Headers
Enter the name and e-mail address of the person sending the email. Enter the subject line.
Attachments
Include any of the form outputs as an attachment. For detailed instructions, see Select Outputs to Include as Attachments.
Email Body
Enter the message. You can enter the message as HTML and text. The text version will be used if the recipient’s email client doesn’t support HTML

Send files by SFTP

Use SFTP to send the results to another application. Click EDIT and enter the following information.

SFTP Configuration
Enter the name of the SFTP server in the testing and production environments.
Outputs
Select the outputs to include. For detailed instructions, see Select Outputs to Include as Attachments.
