Types of fields

Learn about the different types of fields you can add to your Smart Form.

You can use the following fields in your form.

Short Text

Use a Short Text field when you want respondents to type a short answer such as their name or address.

Long Text

Use a Long Text field when respondents are able to type a longer open-ended answer, such as leaving comments or suggestions.

E-mail

Use an E-mail field when you want respondents to enter an e-mail address.

Number/Amount

Use a Number/Amount field when you want respondents to enter a number, such as age, weight or a dollar amount. Values entered in a Number/Amount field are stored as numbers (not text) so they can be used in calculations.

Checkbox

There are a few ways you can use a Checkbox field. Add a single Checkbox if you want respondents to agree with a statement, or use multiple Checkboxes if you want respondents to select answers from a list. Note that respondents are able to select as many checkboxes as they want.

Multi-Checkbox

Use a Multi-Checkbox field if you want respondents to select one or more answers from a list. Answers are shown as boxes and respondents click the answers they want to select.

Legal

Use a Legal field if you want respondents to agree to a legal statement, such as your terms and conditions. Respondents must select a checkbox to confirm that they agree.

Dropdown

Use a Dropdown field if you want respondents to select an answer from a dropdown list. Dropdowns are useful for standard lists such as languages, provinces and countries.

Date

Use a Date field if you want respondents to select a date, such as their birthday, using a calendar.

Combobox/Autocomplete

Use a Combobox/Autocomplete field to help respondents enter information such as an address. A list of suggested addresses appears when the respondent starts typing in the field; the respondent simply selects an address and it’s entered correctly on the form.

Tiered Selector

A Tiered Selector field allows respondents to select an answer from a list, similar to a Dropdown field. However, a Tiered Selector adds more levels to the list. After respondents select an item from the first level, they’re prompted to select an item from the next level. You can add as many levels to the list as you want.
